Below is the link to the 2019-2020 PREP and Sunday Youth Ministry Registration Packet. Please completely fill out all three pages of the permission, emergency medical, and registration form for every child in your family who will be in PREP or Sunday Youth Ministry. The first two pages also serve as the emergency medical form for all religious education and youth ministry events from the current date through to September 30, 2020.

Permission slips for special youth ministry events, Confirmation preparation, and First Communion preparation can be obtained by contacting PJ Ehling.
Please note, there is a different permission slip and registration form for high school students over 18. Please contact PJ Ehling for this form.
